You must not have washboards like we do. You hit the wrong one at too high of a rate of speed, you don't slow down. So, yes, it can easily happen with poor defensive driving beforehand.
We have washboards. Misjudging them, especially in places you drive often and know about them, will lead to the kind of poor braking that causes you to nose into the ditch at the other side of the T intersection, not hit it at 40.
