I feel like to date we haven't been told the whole story as to what the circumstance were to how this happened. When plants are designed and built, engineered plans are submitted to and approved by the Iowa Department of Agriculture to help avoid catastrophes like this occurring. Evidently the events that led to the release weren't foreseen but then again, every circumstance can't always be accounted for, and I am sure occurring over the weekend didn't help.
