A hefty fine is needed along with a detailed investigation with corrective actions identified and implemented.  Agriculture typically gets off easy compared to industry in general when it comes to OSHA and EPA regulation and enforcement.  It's mindboggling how anything of this magnitude could happen.  Would be interesting to know what caused this.
