I think you have some good ideas. I would suggest the largest diameter tube that will fit under the waterer. THe larger the diameter of the tube the more heat will rise to keep the water from freezing Insulated tube down about 2-3 ft and then uninsulated below that to approx 6-8' below the surface. Do not let the waterline touch the tube if possible. If you put insulated pipe down too far you will not be able to get the heat from the ground to rise up.No need for heat tape then, maybe just some near the ground surface. I have some installations in cold free stall barns with 15" X 8' tubes that do not have heaters in them and the most they see is a thin layer of ice on the coldest days, but that depends alot on how many head you have drinking at the waterer.

Make sure you install 2- 8' ground rods underneath the waterers and bond them to the metal frame of the waterer. Use 240 volt heaters in them if possible.
