Railroad locomotives employ just such a device known as an "alerter". It sounds on a regular basis and if the engineer doesn't respond within so many seconds, it will apply brakes and automatically stop the train. This is not only a sleep preventer, but also insures that the engineer has not became physically incapacitated or had some type of medical problem.
