Had several filter gaskets on autos at the dealership stay on the block. Luckily, it didn't feel right when removing the old or installing the new filter that i caught it at that time. Have seen the lube guy blow oil all over the floor from a double gasket. The best one was when he filled a car twice with oil. Really ran like crap and shook something awful.
The worst farming was an all night thrash on a 3208 cat to get a tractor running for the next day. I forgot to tighten the drain plug. Dad was setting up disk when it and showing my cousin how to run when it stalled at idle. Thinking that was odd, I walked over to see a few drops of oil drip out of the pan. Engine trashed. Lesson learned the hard way.
