I can answer your first question.

If you look at the actual gear ratios for a 9 speed and a 10 speed the spacing tells the story.

If you reuse L on a 9 speed in high range you will land in the middle of 3-4 on the low side. If you try to shift from 4 (low side) to L on high side you will find yourself in a lower gear than 4. Try and you will understand.

10 speed has them spaced so that when you reuse 1st on the high side it is 6th gear. Pretty simple.

It's been ages since I drove a 5-4 and I was a kid (perhaps 14) I don't remember the correct patterns. Fact is there is probably more than one way to skin those cats.
