A lot of places will use a better quality of meat in the mix than your conventional ground round meat that has fat added because it's so lean. Ground chuck, sirloin, brisket, short rib and maybe even some fancier cuts at higher end places. Local place has a great hamburger steak and it's wrapped with a piece of bacon like some places do with filets.
