Good replies above.  I have only one suggestion to add.

The BEST thing I've done is bought a set of "Cow Cuff Links (Hobbles)" out of Valley Vet.  $24.95

I've only had to use them on one cow in the past 5 years, but they are worth their weight in GOLD!  Yes is was a real challenge putting them on, had to put cow in chute and tie each leg off to chute in order to put them on, but once they were on -- the cow looked like a fumbling idiot when she even tried to kick. 

At the end of the fourth or fifth day -- that cow was pretty much ready to just stand and let me take them off her without much restraint at all.  She was ready to let anything happen as long as those hobbles come off.

Main thing after putting the hobbles on was the calf HAD to get hungry in order to get past its 'timidness' to approach the cow -- had been kicked just enough to decide that wasn't right.
