Had a '91 that did the same thing. Pulled the temperature controller from the dash and cleaned the edge connector with an eraser.

Problem solved.

If anybody ever has a problem with the blower not running in all speeds on a late model GM truck let me know. Just fixed an '06 Colorado and a '06 Sierra. Both had the same problem.
