I replaced the head and plunger in a IOWA hydrant a couple years back without digging it up. Not sure if thats the brand you have or not, but I was able to use a pipe wrench to remove the head, pull the plunger shaft out, put a new plunger on the shaft, and screwed a new head on.

The hardest part was getting the new plunger to seat into the bottom, it was very tight.
