There are other questions to be answered first. Are you modifying the original frame, or is it long enough to begin with? What type of suspension? What type of hoist? What are the measurements of the current frame, and the condition of the frame?

I would go the whole way, from under the cab to the hinge. If you're talking about an average 20' box (not overwidth or extra tall sides) you can go with an inverted L for the double frame rather than the complete C channel, which may save a lot of time and effort, depending on the type of suspension.
