Couple of things, first I would make sure all the dirt and debris is remove from behind and below the cutterbar. I'm assuming this is a JD 200 or 900 series flexhead. The rod that runs the lenght of the cutterbar that turns the sensor inside the sensing box needs to pivot freely. It could be stuck in the ranges of the second and third settings as you describe.
Also I have found the contacts in the sensor box, well not making contact on all of the three ranges. Take the cover off and be sure that all is well inside. Correct adjustment is nesscessary.

If none of this helps it could be a blown o-ring inside the header height control box, this I have repaired on a 6620 JD. Oil was bypassing and poor header height action resulted. But this would be the last place I would look as per your describtion.
