It could be a torn sheath, an infected sheath, or a broken dick. This first can be fixed somtimes but has about a 40% success rate and then if you overcome that you hope the scar tissue isn't to much let him extend after it's healed. An infected sheath can be fixed with anitbiotics. The last means hamburger.
