Redwood Falls, MN | I've been using this for about 3 years now. It works well which is good since the only other option is dial up. The system my provider uses is an older system but my speeds are in the 350 - 450 range. Good enough for most things I'm doing. There are 2 other providers in the area with newer equipment that offer higher speeds but at a price. Currently I pay $30/ month. I'd need to go to $40 for 512 and not sure what for higher. It's quite reliable.
One word of caution, if you have a choice of providers, some will limit the upload speed to 512 even if your download is faster. It's not an issue unless you do a lot of large upload such as web page work, online backups or upload photos to online sites.
As far as firewalls, you need a good one no matter whether you're on dial up or high speed.
