North Central Iowa | Took a look at and found a few things and now it is working for me (attached).
1. When you use symbols, especially math symbols like * - + /), as part of a text string, Excel goes into default math thinking and doesn't see those as text, but as a mathematical equation. I changed the name of your one product by replacing the / symbol with a space. That seemed to help it.
2. I took out the TRUE/FALSE part at the end of the function as that is only optional.
3. The range that you referenced in your function (,mixes!$A$2:$W$101) included a lot of empty rows. I have seen Excel throw fits and issues like yours when there is a bunch of empty blank rows. When I changed the range to ,mixes!$A$2:$W$15 then things starting inserting the right values.
LOOKUP functions can act very quirky and you have to have everything lined up nicely for them to work effectively.
