se Neb | Back when I was working for a large multi-state manufacturing company, every year we'd get a company-wide survey covering management, facilities, manufacturing process, etc. We were told it was a "blind" survey, but it was passed out to us in an envelope with our names on it. The survey also had a bar code on it. So much for a blind survey. I always expressed my true opinion on it ------after I scratched out the bar code!!
Since they know that you hadn't returned your survey yet, if I were you, I'd be looking at the form to see if there was anything to identify you on that blind survey.
