My wife is a hospice nurse. Rom listening to her, you should get them involved as soon as possible. She has had one patient for 6 months and he has been with the agency for over a year. Her agency will work to figure out the goals, and try to help accomplish thrm. One farmer wanted to get through harvest. They tried. One man wanted to get to church, one more time. If they get involved earlier enough, they can help accomplish those goals.

The benefits that are typically overlooked include them having their own doctor when you need medication. If they are involved, they can get oxygen or hospital bed or anything else along those lines pretty quickly.
