| Hi Von, if I was you I would use Google Forms, course its easy, easily customizable, and would work great for what your trying to do.
I made up a form real quick to show you what it would look like. The first 3 pictures are of the form itself, but the last one is the spreadsheet that all the information entered in the form gets saved to. On your Ipad/Tablet you could download the Google drive app, so you can tap on the form you want and it will automatically open Chrome to the form.
At the end of the year you could pull up Google Sheets on your computer and quickly put all the information on the sheet to you invoices.
