It could be done with Adobe Reader DC if a person really wanted to but I would agree with the others and just create it in Excel and go that route.

You can "markup" PDF's in Adobe reader by using the Comment tool.  Don't know if mine is setup as default anymore but to get to the comment there is a text bubble symbol on the right that opens the comment toolbar at the top.  Then you could select the text box tool and draw a text box in each field and adjust the font/color as desired.  I think it would be better in the long run to use excel but it could be done if you wanted.  

I use this a lot to fill out a form emailed to me so I don't  have to print it out and scan it back.  If I'm on my work computer I use  Bluebeam Revu to edit PDFs, much quicker and more powerful, but $$$.
