You can build any form, database(Library) that you want.

You only need one form, but they call it a Library.
Unlimited entries per library.

The easiest way to explain it, is like this.
Think of your Library as a book.
EVERY page in your book(Library), is the exact same blank FORM that you created.
When you are at a new field, you open up the library, click the +, to add a new entry, and fill out the form that you made.

Here is a snap shot of my Equipment Library.
I created the FORM with what I wanted in it.
and then a snap shot of what shows up, when I want to add another entry.
Each time you create a new entry, the exact same form opens up and you fill in the form.

You are allowed unlimited entries.
Yes, you can make dropdowns to have a selection list of items.
Yes, it will do calculations.

It is a VERY customizable database app.
I know I do not need, or use everything this app is capable of.

Now, for the negative.
If you have never played with a database before, this one, is not easy.
To get the FORM built for your Library is very time consuming.
But, once the FORM is built, data entry is easier then writing on that form you posted.
