I might as well blow my own horn - the Soil Fertility Handbook is exactly what you need. It was written with an Ontario focus, but is broad enough that the principles are applicable to a wide range of conditions. The goal was not to make a specific recommendation, but to help sort out where the different recommendations come from and which ones might fit best for your own situation. There are chapters on Sampling, Analysis, Nutrients, Soil pH and Liming, Organic Nutrient Sources, Fertilizer Recommendations, and Fertilizer Application.

The cost is $35 Canadian, plus shipping. It is in a mylar cover with wire binding, so it can survive travelling around on the dash of the pick-up.

We are actually working on a second edition, which should be out in January. More tables and charts, and updated information, but will be five bucks more.
