Take All used to be a major problem for us in wheat. We always had some cheat grass in the alfalfa and the cheat was a perfect host for take all. Adding potatos to the rotation along with fumigation broke the cycle and could grow wheat again. Wheat following wheat was a sure fired disaster for us. I would think that wheat following grass seed would also have problems. Burning, baling, and plowing will all help. My only recomendation is to try small plots so that you don't have a big disaster. Also if you could get a non host crop into the rotation it sure would help.
