If you open the glumes and see a pink/salmon colored grain (or whats left of it), yes its probably scab. Attached is a photo to confirm.

Its caused by rain/humid conditions at (or slightly after) flowering. Its more common when wheat is planted (especially using no-till) into corn stalks, but there can be huge levels of innoculum in the air during favorable conditions, which are deposited on the grain heads. The fusarium then feeds on the sugar released in the anthers and then migrates into the grains, shutting down flow of moisture and nutrients. Typically all spikelets above the infection site will die, causing a white tipped head as pictured below.

If you have 1-5% scab, then good cleaning AND Seed treating is essential. Keep an eye on your fields becuase if you have 5% today it may be 10% tomorrow. Keeping seed from heavily infected scab fields should be discouraged unless you have no other options for seed.
