Reproduction is the first key. Need pregnancies to get fresh cows. Should be calving 10% of your total herd every month. Milking 100 cows means you should freshen 10 animals per month.

48% heifers, keeping DOA's below 7%, calf mortality below 5% 0-60 days, below 2.5% 61-120 days, and <5% reproductive culls, calving at 22 months will give you the ability to cull 45-50% of your herd annually if you stay a constant herd size.
