I was having the same problem this spring after the dealer put on the new updates. I had to delete everything off the card or put in a new empty card to make it work. What was causing it for me was trying to set up some documentations and trasfer them to the card from apex. Maybe not completing documentation setups? Anyways, i'm not much help other than a new clean card will work.
