One thing we have used where we have a problem with it continuing to wash is to use a loader and fill the ditch with old sod.  New rain will not normally wash that back out and it should grow and save you trying to seed an area prone to washouts.  We had this problem in a couple areas where the county ran a new culvert and dumped it into a field.  I tried seed it and could never get it established so we used sod.
