No way for that to happen in egr cooler. Oil cooler will generally leave oil in coolant due to pressure differential. Probably head gasket, which could be due to a walking liner. Had that issue on our 9230. Ended up with an overhaul and had to counter-bore the block. One morning it wouldn't crank because a cylinder was full of coolant.
