Correlation doesn't equal causation. It's also not a silver bullet answer.
Probably a mix of increasingly sedentary lifestyle, a vehicle/driving-centered civilization, combined with a flood of cheap highly processed food, less social stigma and more accommodation for overweight people, and medication and technology that can improve or elongate life with the mounting effects.
