wish i had taken pictures last month when we did ours. We formed it like a stair. a 2x4 flat and one on edge to form an "L". top the "L" was set to floor grade, bottom edge was screwed to the 2x6 we used for the outside form that was mostly in the ground. the door couldn't close until we removed to forms. we have less than a board thickness gap when closed up. We vibrated it really aggressively to get concrete to flow underneath the "L" form and it turnout out pretty good. Our door was only 20ft so the forms supported themself pretty good. Longer spans might need to be done in 2 pours.
