My opinion.... Even oil being pumped through a small cavity has force behind it. Those little pieces in pic three look to me like they could have come from the jagged places in pic two. One piece gets broken off and breaks off the other piece, pluggers the galley part ways starves rod journal of oil and causes the failure....was the part defective? I say strong chance it was. Good luck in your repair.
