Price of a new ambulance?

Subject: Price of a new ambulance?


Central Illinois
Firefighters Union in SPRINGFIELD is trying to convince mayor and council that the fire department should get into the ambulance business.
I would like to include the approximate cost of a new fully equipped ambulance in my comments on this idea.
Does anyone have an estimate of cost for a new ambulance ?

I'm not directly involved but our local officials are saying that it takes 3 years to get one. They look at their current fleet and try to plan for retirement of old units and replacement pretty far out.

Just got off the board for local hospital that provides all ambulance locally. In 2022 a new one was 335k.

Our district ordered one last fall. 2 year wait/build time. $310,000 for just the ambulance. Probably another $80-90,000 to stock it with a defibrilator, Lucas device, auto load cot, etc.

You are looking at 400k plus equipment. Delivery time for new builds is 2-3 years depending on chassis and body builder lead times. Other things to consider are increased calls that take longer and keep crews tied up requiring more personal for backup ambulance/crew. Reporting requirements/software cost. An effective billing system that knows the in/outs to get the most payment back from insurance or individuals. Some insurance companies/Medicare can take up to a year before you see any reimbursement for service.

Next township over ordered a gas engine ambulance. I believe it was 400 k and 2-3 year wait. I do remember the power cot for it was a extra 60 k. Yes an EXTRA 60,000.

For $60,000 it's most likely a power load. One back injury can quickly become $60,000

The power load cots are also significantly heavier, and pose problems when hauling them up stairs in houses.

Can't comment on the price - except don't be apposed to used, if you are lucky enough to find one (there come up from time to time). The single best thing about having rural / local ambulance (EMT) is dedication. Here in Mn, there are territories set up for Ambulance routes. No cutting into others territory - but, they do have to work together and back each other up. Our locale FD has EMT/Ambulance service (with a territory). They are getting called into cover other territories more often then not. Our FD / EMT service is funded by it service members (several townships formed a fire district), they do an excellent job of budgeting and working within and improvising to meet the different codes to meet the equipment needs. The biggest issue with rural districts is solicitating to get enough help (volunteers) to keep it all going. The ambulance / EMT service offered in our fire district is the best kept secrete of the communities it serves. Absolute BEST SERVICE offered !

Out of curiosity, why does a box truck with shelves cost 400k?

I could understand 400k with all the equipment, but what is so special about a box truck with lights that it costs 400k?

Ours is contracted out , we don’t have our own .

Your regular ( full timers ) are EMT certified, my boy going to fire academy soon . The Emt is required at big departments .

It seems a lot of it is contracted out . , from what I have seen .

None of us are EMT certified . Chief told me let’s try to keep them alive until the pros arrive .

Seems like a big liability trying to have own ambulance.

Hopefully some full timers reply , they see more action in a week than we get in a year .

Anything for medical, a city, or a school may as well be double what a normal guy would figure it’s gonna cost.

I’m convinced most people on the elected boards think with their heart not their head and it’s not their money.
It’s so common place there’s nothing you can do about it now.

Anything for medical use is closer to 10 times the cost for non-medical use. Some of it has to do with the fit/finish/and material, but mostly it's because of the liability cost anyone that produces those products has to have.

The simple answer?  Because it's more than just a box truck with shelves. 

First understand this: I've got a special spot in my head and heart for ambulances.  A friend was a paramedic.  They were transporting a patient to the hospital lights and siren going.  He was in working on a patient in back when it got t-boned by a little pickup that ran a stop sign. He died in an ambulance trying to save someone else.  Those "box trucks" are special to me.

OK getting back on topic.  An ambulance has to comply with regulations from DOT, OSHA, NFPA, on and on and on. There's folks that get paid a lot of money to sit at a desk and write new regs for all these things.

Plus it has to stand up to face ambulance chaser's lawsuits.

I'm not up on all the requirements, but I'm betting there are rules about how each storage area is made, about the glass in the cabinet doors, about how high the steps have to be, about how the oxygen bottles must be anchored and the lines routed, about how the gurney is connected to the chassis when loaded. pages and pages of rules and regulations.  And the builder has to know and comply with all of them.

It's not as simple as throw a Band-Aid® or two in a couple boxes and stick a lawnchair in back.  Get in an ambulance and look around and think about what it takes to build one.

I have, there are alot of retired ambulances in service here as mechanics and glass trucks.

I'm not arguing what it is equipped with, I'm arguing the lone basic shell you get for half a million.

 

No offense, but saying a mid sized pickup killed the occupants doesn't exactly sell the half a million price, if they had survived, then it is a bit more understandable as then it would have to be built and tested to withstand that.

 

Regulations on a mass produced vehicle still isn't really a reason for a standard vehicle from Ford with, from what I recall from our "Ambulance special" pickup, a larger set of alternators, heavier wiring to the back, heavier springs, and a few others miscellaneous things that only raised the price by 5,000 when new, which still doesn't really make sense that the bare empty box, that isn't crash resistant, doesn't have a ROPS or a FOPS, and is a drop on mass produced thing, even with the shelves, cabinets, anchors, and regulations, which after you make one that does meet them doesn't require you to relearn it for every one,

why does it cost 400k alone, and not hold its value?

 

I mean looking at Brindlee, a couple year old under 50k miles ambulance is 100k. If they held their half million value, then it would make sense that there is something super special about it, but at 5 years old and 50k miles, you've lost 4/5th of the value, and any older and any more you're sub 50k, something isn't adding up.

I was just talking to a rural fireman today at the gas station while I put my 45 cents in spare change in their ambulance fundraiser jar. He said around $400,000 and up to 3 years out.

I say spend even a bit more and get air ride suspension.

Our town board hires an ambulance service to cover our area, but last I knew those quotes around 400 K seem close enough to rough budget from. It will likely be 500 fully equipped.

A new unit, ready to go out the door is going to set you back 500K. And that is if you lock in prices now.

I have a complete list of the costs just for the required equipment for the state of Indiana along with prices.

Emergency equipment is stupid expensive.

Our local EMS which is ran by our county ESD buys the Ambulance rated trucks and then swaps the bodies over. I believe he (director) said the current truck is 2 years old and has 250k miles or something crazy like that. He had a new 2024 cab/chassis at the office and that’s what intrigued me to ask. He said they just wear the trucks out too fast so they swap bodies. I am almost certain he said there is a ambulance body builder in Houston that actually does the swap, not 6-8 EMTs between calls out in the bay, that would be great if they did though!

Wow that's alot of miles in a short time, I think in our area they get as many miles in parades as they do saving lives, fire trucks too, I'm glad they are there but boy the cost. And I know I know if it saves just one life, that's how they are usually sold.

I think it really depends on what you buy as far as price. Our local volunteer service that does our transports just got a new Type 1 with lots of bells and whistles and it think it was around $450k. Another local private ambulance runs Type 3 rigs and they were talking somewhere around $320k for new and less than $150k for a remount(putting used ambulance body on new chassis). Just like anything else wants vs. needs plays a big role in the price.

Out how to get a state grant for it .

There may be programs every where ?

Our local rescue squad in the last few years have put a new chassis under our old boxes. We sent them to a company in rock Rapids, Iowa, and they totally refurbish the old box and mount it on a brand new chassis. Massive savings.

A remount is a great option.

The body can be remounted as many times as the structural integrity allows. And there can be some reconfiguration done at that time too.

But the original poster doesn’t have an ambulance to remount.

Last fall our small town (500) started a fundraiser for one. The final purchase quote, which includes the ambulance and the power load system/cot was just shy of $336,000. There was a 10-month lead time before any manufacturers could start on a new unit, so they were looking at 2025 at the earliest to get one.

My buddy has a brand new one sitting here if interested. Bought from insurance. Front end damage but the back end is new. They have taken the drive train out and put in something else. Actually they have two sitting here. Both are insurance wrecks.

I would hope that if you spend that much money for an ambulance, the poor passenger in the back should get a better ride. I had the opportunity to ride about 150 miles laying on I think a board for 100 and some miles on a rough road my back was bad before it started, but it was a lot worse when I got there.
