On a single wire alternator there is a wire from one of the flat posts to the output post to sample the voltage. if that is missing the regulator can't work right because it can't sense the output voltage right. It would be better for that wire to be run to the battery so the regulator set the voltage at the battery, not at the alternator terminal. Then voltage drop in the wiring would be corrected for.

At 12.5 volts without charge, the batteries appear to be OK. If they showed 12.5 volts with the alternator cranking out 16, then I'd say the charging circuit is open or there's a bad cell in one of the batteries which you could easily check by comparing the voltage of the two batteries that are in series. Should be 6.25 each without charging or load and should be 7.1 each or 14.2 when charging and at full charge.
