SW OH | I've seen all of the above and a few more. Too tight, too loose, vibrated loose, stuck relief valve, double gasket, but the one that irritates me the most: When the nipple is loose in the block or oil cooler. It happens when the previous filter was too tight. Loosening the previous filter removes the nipple down to about 1 thread. Then the nipple stays put going deeper into the filter. Most of the time they just leak. Sometimes it will blow the filter off.
